There may come a time when you hire a new attorney or change an attorney to handle your trademarks or applications, or simply want to change the attorney information, like a new address.
The USPTO trademark section has a variety of on-line forms. One such form is called: Change Address or Representation Form. Of course, there must be a 3-letter acronym for this form and it’s: CAR
The CAR (actually Correspondence and Attorney/Domestic Representative Forms) can be found here:

As you can see it can be used for a variety of changes.

Conclusion
While it’s easy to get lost on the USPTO website, these links should take you to the forms you need. If you get the dreaded: 404 error – page not found
then the USPTO has again messed up the website and your best bet is to go here: https://www.uspto.gov/
and in the upper right search for: “Change Address or Representation Form” without the quote marks.
